These are the only reasons a ban appeal would be accepted. Appeals for "apologies" or just admitting you did wrong will be instantly denied.

Reason for ban on record : Faction Hopping
Why do you think you were banned?(what you think the admins thought) : I left my faction because I was not able to enemy. Created a new faction so that I could fight.
Why what is your explanation of this reason? 1. No hacking, cheating, or exploiting
2. No spamming
3. No excessive swearing
4. Don't be an ass use common sense
5. No asking for items or ranks (unless they were donated for)
6. Treat everyone with respect
7. No alt accounts (Tell us if you have someone on the same ip playing with you)
8. No PVP logging
9. Scamming is allowed
10. What admins say goes. Do not argue.
11. Don't use excessive caps.
12. No making new factions to evade enemy territory
13. Macros are not allowed
14. AUTO acrobatic grinders are not allowed
15. No impersonating cops. (5 minute jail time)
Why should we unban you? I was not aware that it was against the rules. I am aware there is no combat logging but I was trying to allow myself to get INTO combat.
What measures will you take to prevent this from happening again? Stay in one faction
How can we trust you again? I read the rules and didn't see anything about the offence.
