Ban appeals are for when we have made a mistake, or there has been a significant change in the circumstances.
These are the only reasons a ban appeal would be accepted. Appeals for "apologies" or just admitting you did wrong will be instantly denied.

Nature of ban(ie, mine craft temp banned or TS3 perm ban) : MCD and TS3
Staff who banned you : Spalw
Staff who dealt with you : Nobody.
Staff who have warned you previously : None.
Reason for ban on record : Ban extended to perm - attempts to circumvent original ban and disrupt the server.
Why do you think you were banned?(what you think the admins thought) : People were disrupting the server after I was banned.
Why what is your explanation of this reason? I am in discord with my friends and they started disrupting the server using my name.
Why should we unban you? I did not tell them, or encourage them to do this.
What measures will you take to prevent this from happening again? I can tell them to stop.
How can we trust you again? I won't get banned.
What else would you like to say to the admins who will review this case? I don't see how this applies to a permanent ban on my part, I wasn't involved in this at all. I didn't tell them to do this.
