Ban appeals are for when we have made a mistake, or there has been a significant change in the circumstances.
These are the only reasons a ban appeal would be accepted. Appeals for "apologies" or just admitting you did wrong will be instantly denied.

Staff who banned you : noodlyattendence
Staff who dealt with you : i dont know
Staff who have warned you previously : nobody
Reason for ban on record : xray first offence
Why do you think you were banned?(what you think the admins thought) : i guess they think i broke into somebodys base and blew it up
What is your explanation of this reason? i was banned
Why should we unban you? i never did blow up anybodys base i promise
What measures will you take to prevent this from happening again? the admins should get better training
How can we trust you again? im gonna be in the army someday i never lie
What else would you like to say to the admins who will review this case? please get better training so i dont get banned again
